Junior ISA Declaration

For your own benefit and protection you should read our Terms & Conditions carefully before committing to an investment. If you do not understand any points please ask us for further information. When you use our services we will take this as acceptance and agreement of our terms, and you will be bound by them.

I apply to subscribe for a Junior ISA (JISA) for the child. The child named in the application will be the beneficial owner of the account.

I declare that:

• I am 16 years of age or over
• I have parental responsibility for that child
• The child does not have a Child Trust Fund account (CTF)
• I will be the registered contact for the JISA
• The child is resident in the UK, or is a UK Crown servant or dependant of a UK Crown servant
• I have not subscribed and will not subscribe to another JISA of this type for this child *
• I am not aware that this child has another JISA of this type
• I am not aware of other JISA subscriptions that will result in this child exceeding the annual limit
• I will not knowingly make subscriptions to JISAs for this child that will result in the subscription limit being exceeded

* If the child has a CTF or another JISA of this type, you can only subscribe to this JISA after you transfer the existing CTF/ JISA to us.

I authorise the ISA manager:

• to hold the child's subscriptions, JISA investments, interest, dividends and any other rights or proceeds in respect of those investments and cash, and
• to make on the child's behalf any claims to relief from tax in respect of JISA investments.
• I agree to the Terms and Conditions and I consent to both the child’s and my own personal data being used in accordance with them.
• I confirm that to the best of my belief the information in this form is true.

I declare that the application form has been completed to the best of my knowledge and belief. I will notify you without delay of changes to these particulars.
